The Freedom of Information Act 2000 provides a right of access to a wide range of information held by public authorities, including the NHS. The purpose is to promote greater openness and accountability.

If you cannot find the information that you require on our website, Publication Scheme or FOI Log first, please complete our online webform below to submit a request for information to North Bristol NHS Trust. All fields with a * next to them are mandatory however, please try and complete as many of the non-mandatory fields as possible as this will allow us to process your request more efficiently.

Please note we do not click on links or download attachments from external sources outside of our organisation. Unfortunately, the NHS is prone to attacks from cyber criminals, some using the FOI function as a means to send malicious content in emails and attachments. We have to protect our organisation by making sure our communications with the public are safe and secure. Therefore, we will not open any attachments sent to us.

In normal circumstances the Trust should be able to;

  • confirm or deny that we hold the information that you have requested and;
  • if we hold it, send it to you within the statutory timescale of 20 working days.

You should note however that the Act defines a number of exemptions which may prevent us complying with either of these duties. We will explain if this is the case in our response and whether or not we have applied the 'public interest test' to our decision. In some circumstances a fee may be payable and you will be required to pay, following a fees notice, before we proceed with your request.

To be a valid request under Section 8 of the Freedom of Information Act, requestors must submit their full name. If a request is submitted under a pseudonym then the request will be invalid and refused.

If you are submitting a request from a company, then please submit the company's full registered name or a name that exists as a legal entity (such as a trading name). Your request will be invalid if you are making a request on behalf of a company and do not provide the company's name. We will contact you to confirm these details and reserve the right to refuse the processing of your request until we receive clarification in order for us to assess fair processing.

If you are submitting a request as a sole trader, please submit the proprietor's name or the company name. Again, your request will be invalid if you are making a request on behalf of a company and do not provide the company's name.

To be a valid request under Section 8 of the Freedom of Information Act, requestors must provide a valid address or email address for correspondence.
